Comments:
An unannounced monitoring inspection was conducted today with the director, who was present with 3 additional staff and 28 children. Two children had emergency medication on site and two staff are MAT certified. Five children's files were reviewed and there has been one newly hired staff since the last inspection and that one file was reviewed as well. For snack the children were to be served apples, animal crackers and water. The fire inspection is dated 9/28/18. Arrival time: 3:45 pm Departure time: 4: 45 pm

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of staff A's file, it was determined that staff A lived in Michigan in the last five years and there was no documentation that the center has sent off for staff A's out of state central registry background check within 30 days of employment. Staff A was hired on 3/4/19.

Plan of Correction: I will check to see if the background check request has been sent off . We will resubmit or check on the status of the background check by 6/7/19.
