Standard #: 22VAC40-665-500-F Description: Based on observation, record review, and interview, the center failed to ensure that a current written list of all children's allergies, sensitivities, and dietary restrictions were maintained in a way that was accessible to all staff who work with children. Evidence: 1. The physical examination in the record of child #1 had an intolerance to a food and an allergy to a medication documented by the physician. This was not on a written list available to staff. Staff #1 also stated that he was not aware of this sensitivity/dietary restriction and allergy. Plan of Correction: The child care director/assistant director will review all children's files and develop a new list of allergies, sensitivities, and dietary restrictions.
Standard #: 22VAC40-665-580-E-1 Description: REPEAT VIOLATION Based on record review, the center failed to ensure that all staff had current first aid and CPR certification. Evidence: 1. Staff #5 first aid and CPR certification expired May 2019. There were five other staff working directly with the children who had current first aid and CPR certification. Plan of Correction: The staff person will attend a first aid and CPR training tomorrow.
Standard #: 22VAC40-665-780-A-3 Description: Based on observation of posted emergency drills and interview, the center failed to ensure that a lock down drill was practiced at least annually. Evidence: 1. There was no documentation observed of a lock down drill practiced for 2018. 2. Staff #1 stated that a drill had not been completed. Plan of Correction: A lock down drill will be completed and documented for 2019.
