Comments:
On May 24, 2019, an unannounced mandated inspection was completed. The inspection began at 3:20pm and ended at 6:00pm . During this time, the Inspector observed 68 school aged children engage in gross motor play, computer time, and movie watching. The Inspector reviewed five staff and five children's records as well as drill documentation and first aid kit contents. The program does not administer medication.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of records, the provider failed to require that each child had a physical examination by or under the direction of a physician within one month after attendance. Evidence: Child (4) did not have documentation in her file supporting that she obtained a physical examination by or under the direction of a physician. Child (4) enrollment date was 8/21/18.

Plan of Correction: This was a copying error. The mother has been asked for information.

Standard #: 22VAC40-185-160-C
Description: Based on observation, the provider failed to provide documentation at least every two years from the date of the first initial screening or testing for tuberculosis. Staff members shall obtain and submit the results of a follow-up tuberculosis screening as stated in subsection B of this section. Evidence: Staff (3) did not have any documentation in his file reflecting a current TB test or screening. The last TB test was completed 1/12/17.

Plan of Correction: The provider will ensure that staff obtains proof of TB screening or testing and that the negative result will be filed in his records.

Standard #: 22VAC40-185-60-A
Description: Based on review of files, the provider failed to maintain children's records, specifically the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached. Evidence: 1. Child (2) was missing complete addresses for both emergency contacts. 2. Child (4) was missing address for one of his/her emergency contacts.

Plan of Correction: Have asked for this.

Standard #: 22VAC40-185-320-B
Description: Based on observation, the provider failed to ensure that rest rooms were equipped with soap, toilet paper, and disposable towels or an air dryer within reach of children. Evidence: The girls rest room was not equipped with an air dryer or paper towels.

Plan of Correction: They were in the building. Other staff could not locate. Janitor was out sick.

Standard #: 22VAC40-185-560-K
Description: Based on observation and staff interviews, the provider failed to sanitize the tables after each use for feeding. Evidence: There were visible crumbs and liquid on the table after the children ate their snack. Additionally, the Inspector asked the staff person if the table had been sanitized after snack and he stated that it had not been sanitized. The inspector observed the children using the tables for various activities after snack.

Plan of Correction: The staff stated that they will clean and sanitize the table today and practice cleaning and sanitizing according to the standard.
