Comments:

The licensing inspector conducted an unannounced monitoring inspection on 05/30/2019 from 9:50am to 1:45pm. The children were observed playing on the playground and playing in centers. The infants were observed having floor play and napping in cribs. A formal observation was completed with the two year old and toddler classrooms. The lunch menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today. Seven children?s records and five employee records were reviewed during this inspection. Medication is being administered and was reviewed. Twelve injury reports were reviewed as well as the center?s injury prevention plan which was updated 04/2019 The center's first aid kit and emergency supplies were inspected. Last emergency drill: 05/22/2019 Last shelter-in-place drill: 04/26/2019 Last fire inspection: 05/15/2019 Last health inspection: 04/16/2019 The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection. Standard #:	22VAC40-185-140-A
Description:	Based on review, the center did not ensure that one of seven children's records contained documentation of physical records before the child's attendance or within one month after attendance. Evidence: The record of child #1 (start date: 9/10/18) had documentation of a physical record dated 10/25/2018.
Plan of Correction:	We will ensure that upon enrollment parents will submit shot record and physical.
Standard #:	22VAC40-185-270-A
Description:	Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. The center's fence surrounding the playground had several protruding nails. 2. The Jr. K classroom had areas with peeling and chipped paint.
Plan of Correction:	1. Maintenance corrected during the inspection. 2. Maintenance will spot paint.
Standard #:	22VAC40-185-330-B
Description:	Based on observation, it was determined that the preschool/school age playground did not have sufficient resilient surfacing underneath and surrounding the climbing play equipment. Evidence: On the preschool/school age playground was a multicolored composite climbing structure. Directly underneath and surrounding the entire structure was mulch as well as decomposed mulch which has turned to mud/dirt. The inspector measured several areas around the structure. The mulch measured approximately 3 to 5.5 inches in depth and not the required six inches.
Plan of Correction:	Mulch has been ordered.
