Technical Assistance:
Annual Virginia health & safety updated training for all staff due March 1, 2020.

Comments:
An unannounced subsidy health and safety inspection was conducted on 06/05/2019 from 10:23am - 1:40pm. A monitoring inspection was additionally conducted as this facility is a licensed child day center. At the time of entrance there were 42 children in care with seven teaching staff and two administrative staff. Children were observed during outdoor play, free classroom play, lunch and nap. Records were reviewed for ten children and four staff.

Standard #: 22VAC40-665-540-A
Description: Based on record review and interview, the vendor failed to ensure that staff shall obtain documentation of a TB screening at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment. Evidence: 1. The results of a TB screening / evaluation was not on file for staff 1 who began employment on 05/26/2019. a. Staff 1 obtained a TB skin test on 05/28/2019 however, she did not return on 05/30/2019 for the results to be read by the medical professional. 2. Administrative staff confirmed that a TB screening / evaluation was not on file for staff 1.

Plan of Correction: Staff 1 was called during the inspection and asked to obtain a TB screening before coming into work.
