Comments:
An unannounced Monitoring Inspection was conducted today from 10:30am to 12:20pm. There were 26 children, infants to 5 years of age, directly supervised by 5 staff. The physical plant, 5 staff records, 4 children?s records, no children's medications and medication authorization records were on site, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, painting, preparing for lunch, eating lunch, and preparing for nap time. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Standard #: 22VAC40-185-40-J
Description: Based on review, the facility failed to ensure that the injury prevention procedures are updated at least annually based on documentation of injuries and a review of the activities and services. Evidence: Reviewed the injury prevention procedures and found that it has not been updated within the last 12 months.

Plan of Correction: Will correct by the end of the day.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility failed to ensure that all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: 1) Observed on the playground a broken plastic tub with handles that have broken off leaving two pieces of plastic sticking straight up. 2) Observed on the playground a riding car that had a broken door latch.

Plan of Correction: Corrected at the time of the visit. Both items were removed from the playground.
