Comments:

An unannounced monitoring inspection was conducted from 10:30 am - 2:30 pm with the center director and staff. There were 32 children, ranging in age from three months to five years of age, present with 6 staff supervising. Children were observed participating in arts and crafts, playing outside, eating lunch and napping. Lunch served today was spaghetti with chicken and tomato sauce, green beans, pears and milk. Four children's files were reviewed. Four staff files were reviewed and in compliance. The last fire drill was conducted 5/31/19 and the last shelter-in-place drill was conducted 9/27/18. The medication log and seven medications and authorization forms were reviewed. Description:	Based on a review of four children's files, the center did not obtain documentation that each child has received required immunizations prior to the child's first attendance. Evidence: the file for child #1 (date of first attendance: 4/21/19) di dnot have documentation of immunizations.
Plan of Correction:	Will ask parent for missing information.
Standard #:	22VAC40-185-140-A
Description:	Based on a review of four children's files, the center did not obtain documentation that each child has received a physical examination before the child's first attendance or within one month after attendance. Evidence: the file for child #1 (date of first attendance: 4/21/19) and child #4 (date of first attendance: 3/29/19) did not have documentation of a physical examination.
Plan of Correction:	Will ask parents for missing information.
Standard #:	22VAC40-185-40-G
Description:	Based on review of four children's files, the facility failed to have documentation of proof of the child's identity and age within 7 days of enrollment. Evidence: Child #1 (date of first attendance: 4/21/19) did not have documentation of proof of identity and age (proof of birth.)
Plan of Correction:	Will ask parent for missing information and will ensure that documentation is received upon enrollment for future children.
Standard #:	22VAC40-185-60-A
Description:	Based on a review of four children's files, it was determined that not all files were complete. Evidence: The file for child #3 (date of first attendance: 6/3/19) did not have a complete home address for mom or dad, work phone numbers for mom or dad, or the name, address and phone number for a second emergency contact. The file for child #4 (date of first attendance: 3/29/19) was missing the name, address and phone number for a second emergency contact.

Standard #:	22VAC40-185-260-B
Description:	Based on a review of documentation, the center did not have an annual health inspection. Evidence: the most recent health inspection report on file was dated 3/27/18.
Plan of Correction:	Called to request an inspection back in March and was told they were short staffed. Will follow up again.
Standard #:	22VAC40-185-510-C
Description:	Based on review of seven medications, the facility failed to follow required procedures to include methods to prevent use of outdated medication. Evidence: the medication log documented that medication with an expiration date of 4/30/18 was given to a child on 5/28/19.
Plan of Correction:	Will return expired medication to parent today.
