Comments:
A complaint was received involving physical plant, rest rooms being in disrepair, insufficient mulch, inadequate staff qualifications and training, ratios, special care provisions/emergencies and equipment. Interviews, observation and a review of record keeping and documentation was conducted and the complaint was determined to be invalid. The one violation was non-complaint related. Arrival time: 7:10 am Departure time: 9:35 am

Description: Based on observation and a review of the attendance log, it was determined that the center's staff failed to maintain an accurate written record of children in attendance. Evidence: Eight children were checked as being present on the attendance log when there were 15 children present in the classroom.

Plan of Correction: We will retrain staff on the importance of maintaining an accurate attendance.
