An unannounced monitoring visit and facility tour was conducted from 3:00pm-3:55pm. At the time of inspectors' arrival there were 41 school aged children in care with 3 staff members. Children were observed participating in outdoor and indoor play, art activity, transitioning between activities and departing procedures. A sample of 5 children's records and 3 on site staff records were reviewed. The Site Leader reportred there were no injury reports since the last inspection and there are no medications being administered. An additional record review was conducted at the administrative office on 5/30/19 due to complete staff records being kept off site. Areas of noncompliance are detailed on the violation notice and were discussed with the Site Leader at the conclusion of this inspection.

Standard #: 22VAC40-185-340-A Description: Based on observation it was determined the center staff did not always ensure the care and protection of children in care. Evidence: 1. Outside on the playground inspector observed child #6(11 years old) laying across the top center cross bar of the swing bay. 2. The bar was between 10-12' off the ground. Plan of Correction: Staff will be retrained on appropriate supervision of children in care. Children will be reminded to only utilize playground equipment for the intended use.
