Consultation was given on documenting on the CPS forms, the date and time OBI was contacted, so staff can continue to work until the completed Central Registry letter is received.
An unannounced monitoring visit was conducted today from 10:00-11:45. All rooms are in ratio with direct care staff. Observed toddler and balanced mixed age groups, the playground area, records, medications, emergency preparation and four new staff records. Lunch to be served today is chicken, sugar snap peas, bread and butter and milk. Rooms are well equipped and maintained with Montessori materials and supplies to enhance the curriculum. Standard #: 22VAC40-185-550-M Description: Based on injury documentation, all required information was not recorded. Evidence: 2 of 10 injury reports were missing information. 1 was missing the method and time the parent was notified and 1 was missing future action to prevent reoccurrence. Plan of Correction: Missing information will be entered on the form and staff will be reminded to complete all information.
