An unannounced inspection was conducted in response to a complaint received on 5/28/19. The complaint was related to the Pre-kindergarten classroom being over ratio on 5/24/19. 2 staff members were interviewed, attendance sheets for children and staff were reviewed. Based on the preponderance of evidence, the complaint in not valid. Violations non-related to the complaint have been cited, and listed in the Violation Notice. Standard #: 22VAC40-185-40-E Complaint related: No Description: Based on record review, staff did not ensure that the center's own procedures were being maintained. Evidence: 1. The teacher/staff attendance sheet is used to record times teachers sign in and out to reflect their time worked in any classroom at the center. 2. The teacher/staff attendance sheet dated 5/24/19 for the Pre-kindergarten classroom was not completed accurately. 3. The teacher/staff attendance sheet showed that Staff # 1 was in the Pre-Kindergarten classroom starting at 1:30pm. There was no documentation to show which additional staff member was present in the Pre-Kindergarten classroom between 2:30pm to 4:05pm to help maintain the ratio of 10 children to 1 staff member. 4. Staff # 1 stated that Staff # 2 was in the classroom with her, but most likely did not sign in at the time. Plan of Correction: Staff has been re-trained to use the staff attendance sheets accurately.
