Comments:
Thank you for your assistance with this unannounced monitoring inspection. This on-site visit was conducted on June 10. 2019 from 10:40 a.m. through 1:30 p.m. There were twenty ?six children in the care of five staff in three classrooms. Five children?s and four staff records were reviewed and updated. No medication is being administered at this time. Areas of non-compliance may be found on the violation notice. Areas observed were: free choice activity, books, journal writing, hand washing/bathroom procedures, lunch, and nap. Interactions between the children and staff were observed as appropriate and the children seem comfortable with the daily routine and schedule. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on the record review of five children's records the center failed to have the physician's phone number for three children as well as a second emergency contact for one child. Evidence: 1. Five children's records were reviewed. 2. Child # 2, 3, and #4 did not have documentation of a physician's phone number. 3. Child # 2 and #4 did not have documentation of a second emergency contact person.

Plan of Correction: Parents will be contacted at pick up and asked to furnish the names of emergency contacts and provide the physician's phone number.

Standard #: 22VAC40-185-260-A
Description: Based on record review, the center failed to obtain an annual fire inspection. Evidence: 1. Last documented fire inspection was dated 10/05/2017. 2. The director confirmed the annual fire inspection had not been obtained.

Plan of Correction: The fire inspection is scheduled for June 11, 2019. A copy will be sent to the licensing inspector once completed.

Standard #: 22VAC40-185-280-F
Description: Based on observation the center failed to label the original contents on one spray bottle. Evidence: 1. One spray bottle in Classroom #1 did was not labeled with its original contents. 2. Staff confirmed the spray bottle was labeled "bleach water."

Plan of Correction: Staff immediately labeled the bottle "bleach water." All staff will be reminded to label any bottle with it's original contents.

Standard #: 22VAC40-185-500-A
Description: Based on observation the center failed to have one child wash his hands after eating lunch. Evidence: 1. One child was observed eating lunch. 2. One child was observed getting up from the table and going to play with the toys without washing his hands. 3. Staff confirmed they did not have the child wash his hands after eating and before going to play with the toys.

Plan of Correction: Hand washing procedures will be reviewed with all staff.

Standard #: 22VAC40-185-560-K
Description: Based on observation the center failed to sanitize one table before the child sat down to eat lunch. Evidence: 1. One child was observed sitting down at a table to eat lunch that had not been sanitized. 2. Staff confirmed the table had not been sanitized.

Plan of Correction: Sanitizing procedures will be reviewed with all staff.
