Comments:
An allegation regarding lack of supervision was received on March 11, 2019 and initiated on the same day. On site investigation was conducted on March 13 and 19, 2019. Telephone interviews were attempted on April 15, 2019. The investigation concluded on April 15, 2019. During the investigation staff were interviewed, attempts to interview parents were made, records and documentation were reviewed and classroom observations were made. The information gathered during the investigation supports the allegation and the complaint is valid. Violation regarding supervision appear on the violation notice. Addition violations in the areas of staff records, hand washing and infant equipment were identified and appear on the violation notice.

Description: Based on observation and interview the center failed to ensure that when supervising children staff will always ensure their care, protection and guidance. Evidence: 1 - Staff 1, 2 and 3 report that child 1 was asked two times not to stand up in a chair in the classroom. However, while standing in the chair the third time the child fell forward and banged his head on the table in front of him. 2 - The chair was not removed from the classroom after the incident as it was present in room on March 13 and March 20, 2019.

Description: Based on record review and interview the center failed to ensure that when children are in regularly ongoing mixed age groups, the staff-to-children ration applicable to the youngest child in the group will apply to the entire group. Evidence: 1 - The attendance documentation for March 7, 2019 shows 15 children present ages infant through five years old with three staff. The ages and number of children present in the classroom require an additional staff person in order for the room to be in compliance with the Standards for Licensed Child Day Centers. 2 - Staff 1, 2 and 3 report that they were the only staff present in the classroom on the morning of March 7, 2019 when child 1 fell out of chair and hit his head on a table.

Description: Based on observation the center failed to ensure that there is one staff member for every four children ages birth to 16 months. Evidence: 1 - On March 19. 2019 there were five children ages 4months through 14months old in care in the infant room with staff 6 from 2pm until 3:10pm. 2 - Staff 6 confirmed that she was alone with five infants in care.

Description: Based on observation the center failed to ensure that staff wash their hands before and after each diaper change. Evidence: 1 - Staff 4 changed the diaper of child 2, then changed the diaper of child 3 without removing gloves and washing her hands first. 2 - Staff 4 confirmed that she did not remove her gloves or wash her hands before changing the diaper of child 3.

Description: Based on observation the center failed to ensure that when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely. Evidence: 1 - Child 4 was placed in a blue infant seat, at the kidney shaped table, without the safety belt being fastened. 2 - Staff 6 confirmed that the safety belt was not fastened.
