Comments:
An unannounced monitoring inspection was conducted on 6/11/2019. The inspector arrived at 8:40 A.M. and departed the program at 1:30 P.M. There were a total of 121 children ages 2-12 years old and 15 staff members present. Staff-to-child ratios were met in each group of children. Licensing Inspector reviewed ten staff's , ten children's records and medication/allergy care plan records during today?s inspection. Observed children arrival, bathroom/hand washing procedures, snack/meal served, group activities inside and outside and self-initiated activities. Discussion with administration: Second helpings when food is served, additional MAT modules, TB before hire for subsidy and staff monitoring playground.

Standard #: 22VAC40-185-270-A
Description: Based on observation playground, the center failed to ensure that all areas and equipment was maintained in a clean, safe and operable condition. Evidence: 1. Observed on the playground, the buildings door's has deteriorating wood that posed a hazard to the children. Also observed exposed screws on the building that could snag the skin or clothes.

Plan of Correction: Administration will work on repairing the door and screw and or replace the building. All staff will monitor.

Standard #: 22VAC40-185-330-C
Description: Based on observation, the center failed to ensure that all ground supports were covered for all play structures. Evidence: 1. The large play structure had exposed ground supports that were around the swings.

Plan of Correction: The administration will work on additional mulch and fluff the already existing mulch back over the ground supports. Staff will monitor. Additional mulch will be ordered if needed after fluffing.

Standard #: 22VAC40-185-560-K
Description: Based on observation of the school age classroom, the center failed to ensure that all tables were sanitized according to the standard. Evidence: 1. Observed in the school age classroom the tables were not sanitized after being used for eating breakfast.

Plan of Correction: The administration will ensure that a sanitizing agent is accessible to staff at all times and staff will be reminded of the procedures for sanitizing tables before and after eating.
