Comments:
A Subsidy Health and Safety Focus Inspection was conducted on 05/20/2019 from 1:00 P.M.-2:45 P.M. to review supplemental health and safety requirements. The purpose of the inspection was to investigate a self-report alleged incident that occurred at the center on 04/23/2019. One child record and three staff records were reviewed, five staff were interviewed, and one classroom observation was conducted. Children were observed during diaper change, one-on-one infant interactions, naptime, and infant feeding. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements tat are not covered in licensing standards are reviewed during the supplemental inspection. The exceptions to compliance are noted on the violation notice. Description: Based on documentation and interview, the vendor did not ensure to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision. When a referral is made for treatment from a medical professional. Evidence: The centers representative made contact with the department regarding a serious injury of a child on 04/27/2019, the injury occurred on 04/23/2019. All vendors are required to contact the department of any serious injury no more than two business days after the injury occurs.

Plan of Correction: In the future we will notify the licensing inspector and complete the on-line report within 48 hours of the injury.
