Technical Assistance:
Technical assistance was provided in the following areas of the standards: Background checks; 22VAC40-185-(4)-270-A (Building maintenance)

Comments:
An unannounced monitoring inspection was conducted on 6/3/19 from 8:30am - 10:30pm. During the inspection there were 63 children ages two months old through five years old in care with 13 staff. Children were observed participating in various activities in the classrooms, playing outside, and eating breakfast. Records were reviewed for six children and seven staff. There was no medication at the facility. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and interviews, it was determined that the facility did not ensure that all areas and equipment of the center shall be maintained in a clean, safe and operable condition. Evidence: 1. The HVAC vent located in the ceiling of the Three's classroom was dirty and needs to be cleaned. 2. Staff #7 (Program Director) confirmed the vent located in the ceiling of the Three's classroom was dirty and needs to be cleaned.

Plan of Correction: The facility responded: The vent will be cleaned by maintenance staff. We will check the vents on a regular basis to ensure they are cleaned.

Standard #: 22VAC40-185-330-B
Description: Based on observation, it was determined that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Evidence: 1. The resilient surface (rubber mulch), around the play structure on the large playground was less than six inches in depth in multiple areas of the fall zone. 2. The resilient surface (mulch), around the play structure on the small playground was less than six inches in depth in multiple areas of the fall zone. 3. Staff #7 (Program Director) viewed the resilient surfacing in the fall zone for the play structures on both playgrounds, and confirmed that it was not compliant with the requirements in the standards of six inches of resilient surfacing.

Plan of Correction: The facility responded:Mulch will be added to each fall zone to maintain compliance with the requirements. Staff will check the level of the fall zone on a regular basis to ensure compliance.

Standard #: 22VAC40-185-500-A
Description: Based on observation and interviews, it was determined that the licensee did not ensure that all children wash their hands with soap and running water before and after eating meals and snacks. Evidence: 1. The Licensing Inspector observe child #6 finish eating breakfast and go directly to playing with toys in the Toddler classroom without washing her hands. 2. Staff #7 (Program director) confirmed that child #6 had not wash her hands after eating breakfast.

Plan of Correction: The facility responded: Staff immediately had child #6 wash her hands as soon as it was discovered. All staff will be re-trained on hand washing procedures.
