Technical Assistance:
The playground is off limits due to the sections of down fencing. There is a load of mulch waiting to be spread to meet requirements for resilient surfacing. Both the fence and resilient surfacing need to be completed before the playground is placed back into use. Discussed nap

Comments:
An unannounced monitoring inspection was conducted from 12:45 p.m. to 3:00 p.m. with the director. There were 57 children, ages 1 year to 11 years old, were under the care and supervision of 6 staff. Children were observed during naptime. Infants and toddlers were sleeping. School agers were observed having free play and preparing for outdoor time. The PreK (four and five year olds) class was observed during transition from nap. Seven children's records and four staff records were reviewed. There are currently no medications on site however. There are Medication Administration Training (MAT) certified staff on site. Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas of the center were maintained in safe, operable condition. Evidence: In the two's bathroom, there was one toilet labeled "out of use". The preschool/school-age playground back section of fencing is down . The infant/toddler playground's front section of fencing is down.

Plan of Correction: A plumber will be contacted regarding the two's toilet. A recent storm caused damage to the playground fence. The insurance company has been contacted. While waiting, the playground is off limits.

Standard #: 22VAC40-185-320-B
Description: Based on observation, each rest area in the center was not equipped with paper towels. Evidence: In the school age room, there were no paper towels in or near the sink area.

Plan of Correction: The paper dispenser came loose from the wall today and is scheduled to be repaired. Paper towels were provided during the inspection.

Standard #: 22VAC40-185-440-E
Description: Based on observation, there was not at least 12 inches of space between each occupied cot. Evidence: In the combined three's and four's nap room, two cots were occupied and observed touching one another. Two additional cots were observed occupied and had approximately six inches of space between the resting children's heads.

Plan of Correction: Staff will be reminded to leave at least 12 inches of space between cots.

Standard #: 22VAC40-185-450-A
Description: Based on observation, not all cots were equipped with bottom covers. Evidence: In the combined three year old and four year old nap room, there were three children observed without bottom covers.

Plan of Correction: Staff will be reminded to use a bottom cover in addition to the top cover. Staff will be reminded to request additional covers from the parents/guardians to have on hand.
