Technical assistance was provided regarding physical plant maintenance and children's records.

An unannounced monitoring inspection was conducted today from 9:30am-12:35pm. There were 89 children directly supervised by 11 staff and all 7 classrooms were in ratio. The physical plant, programming, attendance, menu, 10 staff records, 10 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Several staff are MAT (Medication Administration Training) certified, however, the director reported no medications are on site today. Children were observed eating morning snack which consisted of pretzel thins, orange slices, and water, reading, learning to count using blocks, practicing handwriting, and doing a group SOL preparation activity. Children were also observed eating lunch which was buffet style consisting of various different options including baked pasta, chicken fried rice, burrito, turkey and cheese melt, mashed potatoes and gravy, tortilla chips, and milk. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-160-C Description: Based on record review, 2 of 10 staff records reviewed did not contain documentation of an updated Tuberculosis screening or test completed within the last 2 years. Evidence: 1. Staff #4's most recent Tuberculosis test on file was completed 2/27/2017. 2. Staff #10's most recent Tuberculosis test on file was completed on 5/11/2017. Plan of Correction: Staff #4 has brought me her chest x-ray that was done in July 2018. Staff #10 is no longer employed at the Merit School - last day was 5/24/19.
