Standard #: 22VAC40-185-160-C Description: Based on record review, 2 of 10 staff records reviewed did not contain documentation of an updated Tuberculosis screening or test completed within the last 2 years. Evidence: 1. Staff #4's most recent Tuberculosis test on file was completed 2/27/2017. 2. Staff #10's most recent Tuberculosis test on file was completed on 5/11/2017. Plan of Correction: Staff #4 has brought me her chest x-ray that was done in July 2018. Staff #10 is no longer employed at the Merit School - last day was 5/24/19.
