Comments:
An unannounced monitoring inspection was conducted on 4/30/19 from 2:00 - 3:15 pm to investigate a self-reported incident regarding sight and sound supervision of children. Staff interviews were conducted, documentation was reviewed and a video recording was observed. A violation notice was issued as a result of this inspection. Violations:
Standard #: 22VAC40-185-340-F
Description: Based on staff interviews and the center's video footage, the center failed to ensure that children under 10 years of age shall always be within actual sight and sound supervision of staff. Evidence: On April 24, 2019, at approximately 3:40 PM, child #1 (3 years of age) was left alone and without actual sight and sound supervision of staff for approximately three minutes. The classroom video footage, and staff interviews confirmed child #1 was left alone in the classroom when both staff went outside with the prep class. The video recorded children and staff #1 and #2 exiting the prep classroom, and after the group leaves the classroom, child #1 can be seen coming out of the bathroom, looking around, and then going to the carpeted area. Staff #1 and #2 confirmed they did not realize that child #1 did not go outside with the rest of the class. Staff #3 and #4 found child #1 alone in the classroom, sitting on the carpet looking at a book.
