An unannounced renewal study was conducted today with the director who was present with 19 children with four staff providing supervision. The children were outside on the playground during the inspection. Five children and three staff files were reviewed. One child had medication on site. Please provide documentation by 7/1/19 of staff A's fingerprint background check. Reviewed with director the potential consequences of having a staff on site without all background checks. Arrival time: 9:25 am Departure time: 11 am

Standard #: 22VAC40-185-240-A Description: Based on a review of three staff files and an interview with the director, it was determined that there was no documentation of orientation training by the end of the first day of assuming job duties for staff A and B. Staff A was hired on 6/10/19 and staff B was hired on 5/13/19. Plan of Correction: We will sign off on it today.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on a review of three staff files and an interview with the director, it was determined that there was no fingerprint background check on file for one staff at the time of employment. Evidence: Staff A was hired on 6/10/19 and there is no fingerprint background check on file. Plan of Correction: This standard is under appeal.
