Comments:
An unannounced Complaint Inspection was conducted today in response to a complaint that was received by the Virginia Department of Social Services, Licensing Division. Interviews were conducted with staff relating to allegations of Staff to Child Ratios, Care and Protection, Behavior Guidance. Based upon a preponderance of evidence, the information gathered during the investigation does not support the aforementioned allegations, and the complaint is determined to be Not Valid. Please complete the columns for 'description of action to be taken' and 'date to be corrected' for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office within 10 business days. Complaint related: No
Description: Based on review, the facility failed to ensure that each staff member submits documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment and shall have been completed within 12 months prior to or 21 days after employing. Evidence: Reviewed 5 staff records and found that Staff 5 did not have documentation of a TB screening and has been employed for more than 21 days.

Plan of Correction: Staff is scheduled to get their TB screening completed.

Description: Based on review, the facility failed to ensure that staff records were maintained with the required documentation. Evidence: Reviewed 5 staff records and found that Staff 1, 2, 4, and 5 did not have documentation of reference checks on file.

Description: Based on review, the facility failed to ensure that staff have documentation of Program Leader qualifications. Evidence: Reviewed 5 staff records and found that Staff 1 and Staff 3 did not have documentation of Program Leader qualifications.

Complaint related: No
Description: Based on review, the facility did not have documentation of submitting requests for results of out of state Central Registry checks. Evidence: Reviewed 5 staff records and found that Staff 4 lived out of the state of VA within the last 5 years and did not have documentation of results of a central registry check for that state.

Plan of Correction: Working on obtaining the proper paperwork for the out of state check. Will submit for review as soon as the results are obtained.
