An unannounced monitoring inspection was conducted on 6/4/19 to investigate a self-reported incident regarding behavioral guidance. Documentation was reviewed and staff interviews were conducted. A violation notice was issued as a result of this inspection. An exit interview was conducted on 6/18/19. Standard #: 22VAC40-185-400-B Description: Based on review of documentation and staff interviews, the center failed to use behavioral guidance in a constructive nature that is intended to redirect children to appropriate behavior and resolve conflicts. Evidence: Staff #2 observed staff #1 tell child #1 (age 10) to stop touching a cell phone repeatedly before picking up the phone and tapping the child on the arm with it. Plan of Correction: The staff member involved was terminated effective 6/18/19. All staff will be retrained in positive child guidance on 7/9/19 to ensure the safety and protection of all children. We will also review our safety supervision policies.
