An investigation was initiated on May 08, 2019 from 10:00 a.m. - 10:45 a.m. to investigate a complaint regarding a forbidden action. Interviews were conducted, video recordings, documentation, and staff records were reviewed. The complaint regarding a forbidden action was determined to be valid.

Standard #: 22VAC40-185-410-1 Complaint related: Yes Description: Based on video observation and review of documentation it was determined that staff engaged in a forbidden action. Evidence: On February 5, 2019 Staff A is seen roughly handling child A (dob: 07/22/2016) by picking the child up off of the ground using the child's left forearm and swinging the child to the left side, forcibly placing the child on the ground, and repeating the action to the right side, and forcibly placing the child on the ground. The child is then seen crying while laying on their stomach covering their face into their arms. The LI (Licensing Inspector) observed written documentation in the form of a reprimand which corroborates staff A roughly handling Child A. Plan of Correction: Not available online.
