A joint inspection was conducted with CPS on March 27, 2019 and April 7, 2019. This inspection was initiated from a self-reported incident that was received by the licensing inspector on March 15, 2019. One child's record was reviewed. Six staff were interviewed. Time card documentation, attendance sheets, and the injury report was also reviewed. Areas of non-compliance may be found on the violation notice. Standard #: 22VAC40-185-340-A Description: Based on staff interviews the center failed to obtain ice immediately for a child who had tripped while walking to the playground. Evidence: 1. Six staff members were interviewed. 2. Child #1 was walking with his class to the playground when he tripped and fell by the outside gate. 3. Staff #1 stated she used the two-way radio to ask the director to bring ice to the playground. 4. Staff #2 stated she saw Staff #1 use the two-way radio and to ask the director to bring ice to the playground. 5. Staff #1 said the director ?came to the playground and I told her he bumped his head and could she get ice.? 6. Staff #1 stated she waited ?20 minutes for the director to bring the ice to the playground.? 7. Staff #2 stated the director did not bring the ice to the playground. 8. Staff #3 stated she did remember she was asked for ice but did not bring it. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-185-550-M Description: Based on record review, the center failed to obtain two signatures on an injury report. Evidence: 1. Injury report was reviewed. 2. Only one staff signature was on the injury report. 3. The director confirmed there was not a second signature on the injury report. Plan of Correction: Not available online.
