Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on June 21, 2019 from 11:30 a.m. through 2:00 p.m. There were thirty-one children in the care of five staff. Three new staff files were reviewed and updated. The medication and medication paperwork was reviewed for two children. A serious injury report was received by the Department of Social Services on June 11, 2019 for a injury to one child sustained on the outside playground. Areas of non-compliance may be found on the violation notice. Areas observed were: outside play, hand washing/bathroom procedures, transition, lunch and games. Interactions between the children and staff were observed as positive and the children seem comfortable with their daily routine and schedule. Standard #: 22VAC40-185-340-A
Description: Based on review of injury report and interview with two staff, the center failed to ensure the care, protection and guidance of two children on the outside playground. Evidence: 1. A serious injury report was received by the Department of Social Services on-line June 11, 2019. 2. Child #1 carried a toy on the outside play climbing dome. 3. Child #1 dropped the toy and it hit Child #2, who was standing under the play structure, on the head causing a cut. 4. Child #2's mom was called and was taken to the doctor for medical attention. The child's head injury was "glued" by the doctor. 5. Staff #1 and Staff #2 confirmed the child carried the toy on the play structure and it was dropped and hit Child #2 on the head.

Plan of Correction: Playground policies have been updated to include children will not be allowed to take toys on the playground.
