The licensing inspector conducted an announced mandated initial inspection at the facility on June 20, 2019 from approximately 10:00 am until 11:50 am. The licensing inspector reviewed the required documents, postings, staff records, available equipment, and viewed all areas of use. A review of the emergency preparedness plan, the staff handbook, and the parent handbook was completed. The program will offer snacks to children in care. Medication will be administered. Transportation is not provided. The center has submitted all of the required application materials at this time. Standard #: 63.2(17)-1720.1-B-3 Description: Based on a review of four staff records, one record did not contain an Out of State Child Abuse and Neglect Search Request. Evidence: 1. The Sworn Disclosure Statement indicated Staff #3 (hire date 5/15/19) lived out of state within the past 5 years. 2. The record for Staff #3 did not contain an Out of State Child Abuse and Neglect search request. 3. Administration reported the request had not been sent for Staff #3. Plan of Correction: Per administration, the staff will be contacted and the forms completed and mailed. The documentation will be added to the staff file.
