Comments:
The licensing inspector conducted an unannounced inspection on Thursday, May 30, 2019, Thursday, June 6, 2019 and Thursday, June 13, 2019 in response to a self-reported incident that the Department received on May 22, 2019 regarding supervision and injury reporting. The inspector was on site at the center on May 30, 2019 from 03:02pm to 4:20pm and June 6, 2019 from 10:12am to 11:12am. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The inspector completed additional interviews on June 13, 2019. The preponderance of the evidence gathered during the investigation validates the self-reported incident. Violations were cited regarding injury reporting. There was also a violation found during the inspection regarding staff records. See the violation notice for more details.

Violations:
Standard #: 22VAC40-185-550-M
Description: Based on review and an interview, the center did not ensure to maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. Evidence: The record of child #1 did not contain a record of the injury noticed by the center on 5/16/2019. The center representative informed the inspector that they had an injury report but could not locate it at the time of the inspection.

Plan of Correction: We will be using carbon copy Accident Reports as of 6/24/2019. Parent took the accident report documentation home with them in this instance. Director and Assistant Director will ensure carbon copies are used.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review, the center did not ensure that one of two staff records had documentation of an out of state child abuse and neglect search request by the end of the 30th day of employment. Evidence: The record of staff #2 did not have documentation of an out of state child abuse and neglect search request. Staff #2 had documentation showing that the staff has lived out state in the last five years. The staff has been employed by the center for over 30 days.

Plan of Correction: WCDC has updated our Employee file checklist to include verifying if employee has lived outside the state within the last five years. We will be using all updated sworn statements. Director and assistant responsible for this. An out of state background check was immediately sent to the state of Florida for the employee in question. It was sent on 5/31/2019 and was received back in 6/3/2019. Employee was confirmed to have no criminal record out of state.
