The licensing inspector conducted an unannounced complaint inspection on May 31, 2019 in response to a complaint received by our department on May 1, 2019. The inspector was on site from 10:45 AM to 3:45 PM. Additonal interviews were conducted on June 5, 2019 and June 10, 2019. The allegations of the complaint were in reference to medication administration and the supervision of children. Observations were made at the center, interviews were conducted, and records were reviewed. The preponderance of the evidence gathered during the investigation does not support the allegations; therefore; the complaint is determined not valid. Violations were found in areas that were not a part of the original complaint. See the violation notice for more detail. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the licensing office within 5 business days. Your plan of correction should contain 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Standard #: 22VAC40-185-70-A Complaint related: No Description: Based on a review of four staff records and staff interview, the center did not ensure that two of the requested records shall be kept for each staff person. Evidence: 1. The records for Staff #3 and Staff #4 were not located during the inspection on 5/31/19. 2. During a staff interview on 6/5/19, the licensing inspector was advised the records for Staff #3 and Staff #4 were missing. Plan of Correction: Not available online. Standard #: 63.2(17)-1720.1-B-2 Complaint related: No Description: Based on a review of four staff records, the center did not obtain results of a national fingerprint-based criminal history record check for one staff prior to staff beginning employment. Evidence: The licensing inspector observed the record for Staff #1 (hire date 9/17/18) contained a statement of eligibility based on a national fingerprint-based criminal history record dated 9/25/18. As of 01/22/2018, a statement of eligibility based on a national fingerprint-based criminal history record check is required to be obtained prior to staff working in the facility.
