Comments:
The licensing inspector conducted an unannounced inspection in response to a self-reported concern regarding special services that occurred at the center on May 2, 2019. The Department was notified of the incident on May 3, 2019. The investigation was initiated on May 8, 2019 and completed on June 18, 2019. On May 8, 2019, the licensing inspector was on site from 1:45pm to approximately 5:15pm. Interviews were conducted, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation validates the self-reported incident. See the violation notice for more details. Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the Violation Notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Standard #: 22VAC40-185-40-E
Description: Based on interviews and a review of center policies, the center did not ensure they complied with their own policies and procedures that are required by these standards. Evidence: 1) The facility's infant feeding policies and procedures state: "Bottle Feeding and Infant Mealtime: Before feeding the child, match the label on the bottle to the child's face. Confer with your co-teacher that you have the correct child." 2) During interview on 05/08/2019, Staff #1 reported that on 05/02/2019, she did not check the labeling on the bottle that identified Child #1 (6-month-old) before Child #1 was fed. Staff #1 reported she did not check with a co-teacher if the correct child matched the label on the bottle. 3) During interview on 05/08/2019, Staff #4 reported that on 05/02/2019, Staff #1 "admitted to giving the wrong bottle" that belonged to Child #2 (4-month-old) to Child #1.

Plan of Correction: Per the Center: "-All staff was retrained on Bottle Feeding and KCE policies on name to face bottle verification. 5/21/2019 -Management has acknowledgment forms signed and placed in staff files for licensing and District leader to view during visits. 5/3/19 -For 2 weeks management observed every bottle feeding that occurred with the teacher in question. Began 5/3/19 -Management will complete observations of large group feedings weekly at random and coach staff on any corrections needed. Began 5/3/2019 -Regional Program quality advisor will visit within 90 days to ensure that all pieces are being completed. -Administrative actions will be taken if any staff continue to not follow our feeding standards District Leader and Corporate Team Commitments: -District leader will do unannounced visits to the center to ensure that staff are being retrained and that observations are completed -District Leader will make sure any new managements is trained in proper feeding practices -Program Quality Business Partner will complete a 90 day follow up with teachers and management to ensure comprehension feeding procedures"

Standard #: 22VAC40-185-340-A
Description: Based on interviews and documentation reviewed on 05/08/2019, the center did not provide adequate care and protection for an infant in care that received another child's formula. Evidence: 1) Staff #2 reported that on 05/02/2019, Child #2 (4-month-old) brought four bottles of formula to the center and was fed one bottle at 9:20am and one bottle at 12:00pm. 2) Staff #2 and Staff #3 reported that at approximately 1:00pm, they observed one of Child #2's bottles sitting on top of Child #1's (6-month-old) cubby. Staff #3 reported she observed that Child #2 only had one full bottle of formula left. 3) Staff #1 reported that on 05/02/2019, Child #1 brought four bottles of formula to the center. 4) Staff #2 and Staff #3 reported that at approximately 1:00pm, they observed Child #1 had four full bottles of formula left, but Staff #1 documented on Child #1's daily sheet that Child #1 was fed one bottle at 12:30pm. 5) Staff #1 reported that on 05/02/2019, she fed Child #1 a bottle of formula at 12:30pm, but did not check if the bottle belonged to Child #1. 6) Documentation reviewed shows that Child #1 was fed approximately 3.5 ounces of Child #2's formula on 05/02/2019.

Plan of Correction: Per the Center: "-Teachers and Center management reviewed the licensing standards regarding bottle feeding. 5/3/2019 -Additional resources have been given to staff to help better understanding Infant Policies. 5/3/2019 -Management will continue to have conversations with staff around the importance of proper feeding and infant safety. -All new staff coming in will observe proper feeding procedures by senior teachers. District Leader and Corporate Team Commitments: -District leader will do unannounced visits to the center to ensure that staff are being retrained and that observations are completed -District Leader will make sure any new managements is trained in proper feeding practices -Program Quality Business Partner will complete a 90 day follow up with teachers and management to ensure comprehension feeding procedures"
