The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on May 7, 2019 regarding a concern related to physical plant. The inspection was initiated on May 9, 2019 between 10:42 am to 11:04 am. During the inspection, interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details. Standard #: 22VAC40-185-270-A Complaint related: No Description: Based on observation, interviews with staff and video footage, the center did not ensure all areas and equipment of the center, inside and outside were maintained in a a clean, safe, and operable condition. Evidence: 1. Interviews with Staff#1 and Staff#2 confirmed the main entrance door at the facility not regularly closing properly and a sign being placed on the door as a reminder to make sure the door is closed. 2. Observation of the center's main entrance door in which the door was slightly ajar when closed by staff. Staff#1 pulling the door to make sure close it completely during operation hours. 3. Video footage (Exhibit A) of the main entrance door at the facility. Plan of Correction: The center's director reported the door is fixed now 5/2019
