Comments:
An unannnounced monitoring inspection was completed on 7/3/19. There were 42 children present with five staff directly supervising and additional volunteers/administrative staff at the center to assist if needed. Five newly enrolled for the summer children's records were reviewed and five staff records were reviewed. No medications are administered at the center. The inspector arrived at the center at 8:00 am and departed at 11:00 am. The children were observed during arrivals, during gym time, during computer time, and during breakfast. There was discussion with the program director about the following: the summer camp application permission to apply sunscreen and insect repellent adverse reactions, staff sitting at meal times, practices for hand washing or using disposable wipes after eating.

Description: Based on observation and interview, the center failed to ensure that food brought from home was labeled and dated. Evidence: 1. There were two lunch bags observed and other individual foods in containers observed. The foods were determined to be from home. There was no name or date on the food items or lunch bags.

Plan of Correction: The person who is at the sign in desk will make sure all food from home is labeled and dated.
