Today we discussed staff tone of voice.

A focused monitoring inspection was conducted today to review the following standards operational responsibility, supervision, behavior guidance, forbidden actions and ratios. While there were not any violations found in these areas of standards violations in other areas were observed including physical plant, programs and special services. These violations were discussed and reviewed during and at the conclusion of the inspection on 6/20/2019 with the owner/program director and assistant program director.

Standard #: 22VAC40-185-270-A Description: Based on observation and interview the center failed to ensure that areas and equipment are maintained in safe condition. Evidence: 1 - Child 1 is sleeping in a portable, half size, metal frame crib. However, the sheet on the crib mattress appears to be too big for the mattress as it it bunched up at one end of the mattress posing an entanglement hazard and unsafe sleep space. 2 - Staff 1 and 2 stated that the parent had provided a crib sheet for a full size crib. Standard #: 22VAC40-185-440-J Description: Based on observation and interview the center failed to ensure that occupied cribs have the proper spacing. Evidence: 1 ? Child 1 and 2 are sleeping in adjacent cribs however, there is less than 12 inches of space between the two cribs. There is approximately 2 inches of space between the two occupied cribs. 2 ? Additionally, the crib for child 2 does not have the required 30 inches of space on either of service sides. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-185-560-K Description: Based on observation and interview the center failed to ensure that tables are sanitized before and after meals. Evidence: 1 - At approximately 10:55am a group of children were observed playing with toys on the lunch tables closest to the entrance of the eating area. At approximately 11:00am, after the first group had left the eating area a second group of children entered the space and sat down at the same tables. The children were served lunch on the tables without the tables being sanitized before the meal was served. 2 - At approximately 11:05am staff 3 stated that she was not aware that children from the previous group had been playing on the tables that she had already cleaned and sanitized in preparation for the next group.
