63.2(17) License & Registration Procedures

Technical Assistance:
The issuance of a Conditional license remains pending at this time until background searches are complete, a building inspection request form and a local fire inspection is completed and submitted to Division of Licensing for review.

Comments:
An announced Initial inspection for licensure was conducted this date. Walk thru of all 12 classrooms and both playgrounds was completed. Children's records, staff records, background checks, staff training and certifications were reviewed today. Medications and topical ointments on site were inspected. The center was closed today due to a planned holiday. No children or staff present during the inspection. Licensing Inspector met with representatives from Guidepost to conduct this inspection. As discussed, please forward the local inspections as required for review. This inspection began at approximately 10am and ended at 12pm. Thank you for your cooperation today. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation of the classrooms, one area was found not maintained in a safe condition. Evidence: The carpet area located near the exterior wall of Classroom 6 was torn and frayed and determined to be a potential tripping hazard.

Plan of Correction: We will repair this area.

Standard #: 22VAC40-185-280-B
Description: Based on observation of the classrooms, hazardous substances were found unlocked. Evidence: Air fresheners were found unlocked in both bathrooms in Class #6.

Plan of Correction: These items were locked during the inspection.

Standard #: 22VAC40-185-320-B
Description: Based on observation of the classrooms, one bathroom was not equipped with soap. Evidence: The second bathroom in Classroom #11 was not equipped with soap for the children to use.

Plan of Correction: We will ensure that soap is in both bathrooms for this classroom.

Standard #: 22VAC40-185-330-E
Description: Based on observation of the large playground, the sandbox was not covered. Evidence: The sandbox located on the preschool playground was not in use and was found not covered.

Plan of Correction: We will cover the sandbox today and will remind staff to ensure that it is covered when not in use.

Standard #: 22VAC40-185-520-A
Description: Based on review of topical ointments on site, one ointment was kept beyond the expiration date. Evidence: One topical ointment located in Classroom #6 expired in 2018.

Plan of Correction: A review of all topical ointments will be conducted and any topical ointments that have expired will be disposed of or returned to the parents.

Standard #: 63.2(17)-1721.1-B-3
Description: Based on the Initial application for licensure, all required background searches were not completed. Evidence: Staff #1 and #2 are listed as agent/applicant on the initial application. Out of state Central Registry search results have not been obtained.

Plan of Correction: The search requests were submitted and returned due to an address protocol. The search requests have been resubmitted and the results will be forwarded to the Division of Licensing for review.
