Comments:
A complaint inspection was conducted form 9:55am until 11:00 am in response to a complaint that was received by the licensing office 06/18/2019. After interviewing staff and reviewing all available facts, it was determined that the complaint allegation was not valid. Non complaint related violations were cited on the Violation Notice. Standard #: 22VAC40-185-270-A
Complaint related: No
Description: Based on observation, the center failed to keep areas and equipment of the center maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. Evidence: 1) Paint has chipped off the wall in the open play space on the second floor. 2) Paint has chipped off the wall in classroom 7. 3) Water damage on door to classroom 7. 4) Paint has bubbled and sagged in the upper corner of the open play space on the second floor. 5) Paint has bubbled and sagged in the upper corner of classroom 7. 6) Water damage to ceiling on the first floor. 7) Ceiling damage in the open play space on the second floor. 8) Ceiling tile damage in K-2 classroom.

Plan of Correction: We have had an independent contractor do a diagnostic of the roof and its contents. It has been determined the gutters need to be cleaned. We have been advised to schedule a monthly maintenance plan to ensure all aspects of the roof and its contents remain in proper working order. To date, the gutters have been cleaned and a monthly maintenance plan has been established.

Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: 1) Hand sanitizer was not kept in a locked place using a safe locking method that prevents access by children in the Jr. Toddler Classroom. 2) White board cleaner was not kept in a locked place using a safe locking method that prevents access by children in the Jr. Toddler Classroom.

Plan of Correction: All substances have been removed. Staff has been re-trained in the areas of Safety and Accountability and Health and Safety.

Description: Based on observation, the center failed to ensure that the most recent inspection of the facility was posted. Evidence: The most recent inspection from May 15, 2019 was not posted.

Plan of Correction: The most current violation notice has been posted.
