An unannounced monitoring inspection was conducted on July 15, 2019 from approximately 11:55 am to 2:20 pm. Three staff were present with seven children at the onset of inspection. Five staff and five children records were reviewed. Medication is not being administered.

Standard #: 22VAC40-185-70-A Description: Based on review, facility did not ensure 1 of 5 staff records has documentation of required information. Evidence: The record for staff #1 did not have documentation of start date of employment, two reference checks and emergency contact name/address/phone number. Information was verified by the owner/administrator. Plan of Correction: Owner will have staff fill out the staff information sheet and re-do the reference checks.
Standard #: 22VAC40-185-210-A-4-a Description: Based on review and interview, facility did not ensure 1 staff designated as program leader in staff records has qualifications for the position. Evidence: The record for staff #3 (start date of employment May 16, 2019) designates the individual as program leader. Training certificates for the position show twenty-four hours of training were completed from September 23, 2016 through August 6, 2018. The trainings were not completed within six months prior or one month after beginning in the position. Information was verified by the owner/administrator. Plan of Correction: Staff will be demoted until the training is current.
Standard #: 22VAC40-185-340-D Description: Based on observation and review, facility did not ensure there is a program leader qualified person in each group of children. During designated rest time an aide may be with children but the individual must be at least 18 years of age. Evidence: Staff #5 was observed with the children in the 2's and preschool classroom during nap time. No other staff was present in the room. Staff #5 is not at least 18 years of age. Information was verified by the owner/administrator. Plan of Correction: A qualified staff person will be in the room with her at all times.
