Comments:
An unannounced other inspection was conducted in response to a self-reported incident that occurred at the center on 06/21/19. Interviews were conducted with staff , records were reviewed and observations of the program were made relating to the report. Any violations found in relation to the self-report, are noted in the inspection.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on a self report, a record review and interviews, the center did not ensure the center's activities were maintained in compliance with their own policies and procedures. Evidence: 1. The center's policy manual states in Section B: Food Safety and Feeding (Version 5), that when there is a child with a food allergy, "Staff who come into contact with the child must demonstrate competence in and implement measures to prevent exposure to the specific food or item which the child is allergic to." 2. On 06/21/19 during afternoon snack, Child #1 was served and then ingested a documented food allergen. The cook prepared child #1's afternoon snack separately and labeled it with the child's name, however, staff #1 and #2 did not serve the the separately prepared food item on the lunch cart, but served child #1 a snack that was on the menu. Child #1 took a bite and ingested a bite of food containing the allergen. The program director confirmed both staff #1 and #2 were new to the child's classroom and had recently gone over child #1's food allergy plan prior to them working in the room.

Plan of Correction: The following correctiove action was provided by the center: Staff involved were retrained on the center's food allergen policy and allergies in the building on 6/22/2019. All school staff were verbally retrained on our allergen policies 6/22/2019. Staff are routinely (monthly/with new enrollments) updated on food allergens for the entire enter to prevent exposure. This will be completed on 7/17/2019. Management will ensure that all staff are aware. Any violations are handled as per company policy. DM will inspect on future monitoring visits.
