Comments:
An unannounced monitoring inspection was conducted on July 17, 2019 from approximately 11:55am-1:40pm. Upon arrival to the facility, there were 98 children present with 10 staff members. There were 52 children on a field trip. Additional staff were on site for administrative and support purposes. The children were observed having lunch, washing their hands and watching children's programming. Five staff records were reviewed. Areas of noncompliance are identified on the Violation Notice. Exit interview conducted with the Owner.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review, in four of five staff records reviewed, the licensee did not ensure that each staff record contained all required information. Evidence: The owner confirmed that there was no documentation of job titles and orientation training for Staff #2, Staff #3, Staff #4 and Staff #5.

Plan of Correction: The records will be updated today.

Standard #: 22VAC40-185-430-K
Description: Based on observation and inspection of the facility, the licensee did not ensure that provisions shall be made for an individual place for each child's personal belongings. Evidence: The staff member verified that there was not an individual place for each child's personal belongings as approximately 10 bookbags, four lunch containers and a jacket were observed piled on top of each other on the floor in a school age classroom. (Exhibit A)

Plan of Correction: Owner will purchase baskets as their individual space.

Standard #: 22VAC40-80-120-E-6
Description: Based on observation and inspection of the facility, the licensee did not ensure that a copy of any special order issued by the department shall be posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations. Evidence: The owner confirmed that the special order issued by the department was not posted in the facility.

Plan of Correction: The special order was posted during the inspection.
