Comments:
A Subsidy Health and Safety inspection was conducted on (07/12/2019) to review supplemental health and safety requirements. The inspector arrived at 9:45am and departed the program at 3:20pm. There were 28 children present with six staff supervising the children. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-540-A
Description: Based on file review, the provider failed to require staff to be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. Evidence: Staff number (2) failed to have documentation of a TB screening 30 days or less prior to coming into contact with children at the center.

Plan of Correction: The provider plans to obtain a new TB test/screening.
