Thank you for your time and assistance with the unannounced monitoring inspection conducted today, July 22, 2019. The on-site inspection was from 11:35 a.m. to 3:00 p.m. On this date, at the time of arrival, there were 28 children present in three classrooms and in the care of eight staff members. There were several additional staff members on-site to assist if needed. The total enrollment for the program was 36 children. The exception to compliance was noted on the violation notice and the risk rating for the violation was documented on the supplemental page. There were five staff members' records and five children's records reviewed. The medication and medication paperwork were reviewed for two children. Some items observed/reviewed included: infant care and feeding, diaper changing procedures, nap time, indoor large motor and imaginative play, classroom exploration by the children, application of sunscreen, potty tries, injury reports, evacuation and shelter-in-place drills, diaper ointments and parent authorization, and liability insurance. Standard #: 22VAC40-185-130-A Description: Based on review of five children's records and verification from the director, the center did not obtain documentation of one child's immunization record before the child's first day of attendance. Evidence: 1. The record for Child #1 contained an immunization record dated 08/28/2018. Date of enrollment for Child #1 was 08/24/2018. Plan of Correction: The staff will ensure that for children enrolling in the future, they will obtain an immunization record before the child attends the center.
