Comments:
An unannounced Monitoring Inspection was conducted today from 10:05am to 1:20pm. There were 154 (infants 12) directly supervised by 24 staff. The physical plant, 6 staff records, 6 children?s records, children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing with water play, eating lunch, and preparing for and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Standard #: 22VAC40-185-130-A
Description: Based on review, the facility failed to ensure that they obtained documentation that each child has received immunizations required but the State Board of Health before the child can attend the center. Evidence: Reviewed 6 children's records and found that Child 1 did not have documentation of an immunization record.

Plan of Correction: Will speak with parents and obtain documentation as soon as possible.

Standard #: 22VAC40-185-40-J
Description: Based on review, the facility failed to ensure that the injury prevention procedures shall be updated at least annually based on documentation of injuries and a review of the activities and services. Evidence: Reviewed the injury prevention procedures and found that it had not been updated within the last year.

Plan of Correction: Will be completed by the end of the week.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: Observed in the following classrooms, peeling paint on the walls, accessible to children.: The gym, Math and Science room and Transitional Kidergarteners.

Plan of Correction: Management has already contacted maintenance and it should be corrected by the end of the week.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility failed to ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: 1) Observed bleach water and soap water in an unlocked cabinet, on the floor, in Summer Camp B. 2) Observed bleach water in an unlocked cabinet in the Math and Science room.

Plan of Correction: Corrected at the time of the visit.

Description: Based on observation, the facility failed to ensure that electrical outlets have protective covers. Evidence: Observed in the Summer Camp C room, 4 uncovered outlets on a surge protector.
