Comments:
An unannounced self report inspection was initiated today, 4/5/19. Interviews were conducted with staff and children to determine compliance with the standards. A violation notice was completed regarding the violations found during this inspection. Please provide a plan of correction for each violation cited. Should you have any questions contact your inspector.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on interviews and review of procedures required by these standards, the licensee did not demonstrate operational responsibility for staff following the center?s own transportation policies and procedures. Evidence: 1. Center transportation policy states: "When the bus route is completed at the end of the day, the monitor will walk the bus twice checking all seats from back to front and from front to back then exits the bus." 2. "The driver then walks the bus from front to back and from back to front. They will then move the sign from? full? to ?empty? and lock and secure all doors and windows." 3. Staff #1 & #2 stated that they sat on the bus a few minutes and talked before exiting. The staff stated that they did not do a walk-through of the bus when it was parked, as the center?s policies require.

Plan of Correction: All staff members are retrained, focusing on Active Supervision on Buses, the CVCA Transportation Plan, and the No Child Left Behind Policy. All staff members will accurately and truthfully complete required transportation and supervision paperwork, reflecting training received. All staff members will follow the No Child Left Behind Policy to ensure that all redundant checks are completed to ensure that no child is left unsupervised. Supervisory staff members conduct on-board bus monitoring to ensure that policies are being implemented. Transportation plan revised to include a walkthrough by a third person at the end of each bus route; third person then initials bus roster and records time of check. If a route ends in a location where a third person is not available, monitor will call main office, report that bus has been checked and is empty, and record the time of the call in place for third person's initials.

Standard #: 22VAC40-185-340-A
Description: Based on interviews, observations and a review of videotaped footage the center did not always provide care and protection to a child they were supervising. Evidence: 1. Staff #1 and Staff # 2 who provided transportation to Child #1 on April 5, 2019 left Child #1 (5 years old) on the bus when they parked the bus at the parking location, turned off the ignition, exited the bus and went inside a building where the bus was parked. 2. Staff # 1 and staff # 2 stated that they were not aware that they had left Child #1 on the parked bus until: the person, designated by the parent for the center to return the child to, came to the building where staff had parked the bus to pick up Child #1 at approximately 1:00 p.m. 3. At approximately 1:05 p.m. another individual came to the building where the staff were and reported that a child had been found by school personnel of an elementary school located across Route 11 from the parked bus location.

Plan of Correction: Staff #1 & 2 terminated from employment. All staff members retrained, focusing on Active Supervision on Buses, the CVCA Transportation Plan, and the No Child Left Behind Policy.

Standard #: 22VAC40-185-340-F
Description: Based on interviews, observations and a review of videotaped footage a child under the age of 10 years was not always within actual sight and sound supervision of staff. Evidence: 1. On April 5, 2019 staff providing transportation to children left Child # 1 (5 years old) alone and out of actual sight and sound supervision on the parked center?s transportation bus. 2. Child #1 was alone and out of sight and sound supervision for approximately 20 minutes and, during this time; child#1 exited the parked bus and walked across a well-traveled 40 mph two-lane Route 11 highway. 3. While crossing the highway, child #1 walked out in front of a moving vehicle traveling on the highway. The videotaped footage shows the vehicle applying breaks as the child crosses the road in front of the truck. 4. The videotape shows the driver of this truck passing the elementary school and then turning around in a driveway next to the schools parking lot. The driver is seen on the videotape, pulling into the schools parking lot and going into the building. 5. The videotape shows approximately 11 vehicles passing child #1 as she walks along Route 11.

Plan of Correction: Staff #1 & 2 terminated from employment. All staff members retrained, focusing on Active Supervision on Buses, the CVCA Transportation Plan, and the No Child Left Behind Policy.

Standard #: 22VAC40-185-580-A
Description: Based on interviews, observations and a review of videotaped footage the center staff did not maintain responsibility of a child from the time the child boarded the transportation vehicle until the child was returned to the parents or person designated by the parent. Evidence: 1. Staff #1 and #2 used a center?s transportation bus to pick up child#1 at McCready Head Start Center on April 5, 2019 at approximately 11:45 a.m. and transported child #1 from McCready Head Start to another location/building where the bus was parked. 2. Staff #1 and #2 stated or reported: a. They did not drop off the child at the child?s normal stop on April 5, 2019 b. They left child #1 on the bus when the bus was parked c. They became aware that Child #1 had been left on the parked transportation bus when the person, designated by the parent for the center to return the child to, arrived at the head start center to pick up Child #1 at approximately 1:00 p.m.

Plan of Correction: No Child Left Behind Policy revised to include explicit steps to be followed at the end of the day to ensure no children remain on bus. Staff members trained on updated No Child Left Behind Policy. At hire (and yearly thereafter) staff members will sign the Code of Conduct acknowledging supervision expectations. All staff members will receive training focusing on Active Supervision, the CVCA Transportation Plan, the CVCA No Child Left Behind Policy, and the CVCA Playground Safety and Supervision Plans; this training will include all changes to policy and procedure. Scheduled monitoring activities will reflect regular monitoring of classrooms, playgrounds, and buses by supervisory staff.

Standard #: 22VAC40-185-580-K
Description: Based on interviews, observations and a review of videotaped footage staff did not verify that all children had been removed from the transportation vehicle at the conclusion of any trip. Evidence: 1. On April 5, 2019, Staff #1 & 2 stated that they: a. had provided transportation to child #1 on the transportation bus b. did not verify that all children were removed from the bus at the conclusion of the transportation trip; did not conduct a walk-through of the transportation bus. c. left Child #1 (5 years old) on the transportation bus.

Plan of Correction: No Child Left Behind Policy revised to include explicit steps to be followed at the end of the day to ensure no children remain on bus.
