Comments:
The licensing inspector conducted an unannounced renewal inspection on Wednesday, July 31, 2019 from approximately 9:45 AM until 2:30 PM. The following ratios were observed with a total of 90 children present during the inspection: Infants - 10 children with 3 staff Toddlers - 7 children with 2 staff Twos - 16 children with 2 staff Preschool - 13 children with 2 staff Pre-K - 13 children with 2 staff School Age - 15 children with 1 staff The children and staff were engaged in a variety of activities while the licensing inspector was on the premises. Children were observed playing outside, having water play, building with blocks, playing in the dramatic play center, singing songs, having books read to them, working puzzles, taking bathroom breaks and washing hands, eating lunch and resting at nap time. All areas of the center including classrooms, hallways, kitchen, bathrooms, vehicles and outdoor play area were inspected. The center was equipped with toys and supplies and items were available to the children. The following information was reviewed: injury reports, injury prevention plan, infant feeding schedules, daily attendance, infant daily sheets, practiced emergency drill logs, liability insurance policy, parent agreements, transportation documents, field trip permission forms, staff training information, required postings and menu. The center?s first aid kit and emergency supplies were inspected. Medication is administered, and medication and medication authorizations were reviewed. During the inspection, nine children's and nine staff records were reviewed. Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not ensure all areas and equipment were maintained in a clean, safe and operable condition. Evidence: The frame surrounding the door to the boy?s bathroom in the School Age classroom is deteriorating and contains peeling paint and jagged edges.

Plan of Correction: We will put in a call to our maintenance person and have the frame repaired.

Standard #: 22VAC40-185-520-B
Description: Based on observation, documentation review and interview, the center did not ensure written parent authorization noting any known adverse reactions was obtained for sunscreen for three children. Evidence: Authorization forms for three separate sunscreens belonging to children in the Twos classroom could not be located during the inspection.

Plan of Correction: We will have the parents complete another authorization form if the originals cannot be located.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review of nine staff records and interview, the center did not obtain satisfactory results of the finger-print based national criminal background check by 09/30/2018 for one staff hired prior to 1/22/2018. Evidence: The record for Staff #9 (hire date 09/28/2017) contained satisfactory results of the finger-print based national criminal background check dated 12/18/2018. A member of management stated there was a mix up with the process. Staff hired prior to 01/22/2018 was required to have the results of the national criminal background check by 09/30/2018.

Plan of Correction: We require all new staff to have a finger-print based national criminal background check prior to starting. Staff hired before 01/22/2018 have the results of the background check prior to 09/30/2018.
