Technical Assistance:
According to the American Academy of Pediatrics crib sheets should fit the crib mattress snuggly with no excess fabric bunched which can present serious hazards. Make sure staff are consistenly documenting the time of day parents are informed of injuries/accidents. Maintaining an accurate attendance in each classroom at all times is needed to assist in maintaining supervision. When children move in and out of a classroom the attendance needs to accurately reflect who is present in each room at all times. The center is not using the two composite structures on the playgrounds. Please clearly mark both structures off limits with caution tape and sinage so that it is clear that it is not being used by the children.

Comments:
An unannounced renewal study was conducted today with the director, who was present with 64 children with 12 staff providing supervision. Fourteen children and two staff were on a field trip to a local indoor gym. During the walkthrough the 3 and 4 year old classrooms were outside enjoying water play while the other children inside were engaged in a variety of activities to include learning letters, listening to stories. Six staff and eight children's files were reviewed as well as six medications for five children. A sample size of 12 injury/accident reports were reviewed. Standard #: 22VAC40-185-80-A
Description: Based on observation and reviewing the attendance in the classroom, it was determined that the center is not maintaining an accurate attendance log to reflect children present in the classroom at any given time. Evidence: Five children were marked as present on the attendance log in the Infant room ( 10 months and older) but 4 infants were actually present. There were 6 children marked as present in the young toddler classroom but 7 children were actually present.

Plan of Correction: I will retrain all staff on the need to keep accurate attendance.

Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined that the area inside and out was not maintained in a safe or operable condition. Evidence: The tricycle on the playground was missing handle bar grips which presented finger entrapment hazards. In the toddler and two year old bathrooms there was an excessive amount of clutter such as boxes, toys, clothing, pampers which obstructed movement in an out of the bathrooms

Plan of Correction: We will remove the clutter in the bathrooms by 7/26/19 and we removed the tricycle from the playground- 7/25/19.

Standard #: 22VAC40-185-280-B
Description: Based on observation, it was determined that hazardous substances were not locked. Evidence: In the employee bathroom next to the schoolager classroom as well as in the bathrooms in the two year old and toddler classrooms were Bleach, Lysol Clorox and Disinfectant sprays and solutions unlocked on shelves. The cleaning agents in the toddler bathroom was on the floor and accessible to children.

Plan of Correction: The hazardous substances were locked during the inspection.

Standard #: 22VAC40-185-330-B
Description: Based on observation, it was determined that a piece of outside play equipment that had moving parts and which a child could fall from was not under resilient surfacing. Evidence: The see saw on the playground was on top of the bare ground and not under any resilient surfacing.

Plan of Correction: The see saw will be removed from the playground.

Standard #: 22VAC40-185-510-E
Description: Based on observation and interview, it was determined that the medication, Proair Inhaler for child A did not have a prescription label from the pharmacy.

Plan of Correction: We will talk with the parent and obtain the original prescription box/label.

Standard #: 22VAC40-185-510-J
Description: Based on observation and interview, it was determined that the medication box is not kept locked. Evidence: The medication box is stored unlocked on top of a cabinet in the office.

Plan of Correction: We will obtain a lock box.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of staff files and an interview with the director, it was determined that one staff did not have a Central Registry finding within 30 days of employment. Evidence: Staff A was hired on 6/3/19 and the center did not get the Central Registry finding back unitl 7/23/19 which is more that 30 days from hire. The center failed to mail the background check off within 7 business day.

Plan of Correction: I will remember to follow the timely submission guidelines and to check up on the status of the background check as required.
