Technical Assistance:
Technical assistance was provided in the following areas of the standards: Background checks; 22VAC40-185-(3)-210 (Program leader qualifications); 22VAC40-185-(4)-260 (Annual inspections); 22VAC40-185-(7)-550 (Emergency drills)

Comments:
An unannounced monitoring inspection was conducted on 7/9/19 from 2:30m - 4:15pm. During the inspection there were 23 children ages four to five years old in care with five staff. Children were observed participating in various activities in the gym, eating snack, and during rest period. Records were reviewed for five children and five staff during the inspection. There is no medication at the facility. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of five children's records and interview, it was determined that the facility did not ensure that they maintain and keep at the center a complete record for each child enrolled that contains all required information. Evidence 1. The record for child #1 did not indicate whether or not the child had previously attended child care. 2. The record for child #3 did not contain phone number for the father's employer, and did not indicate whether or not the child had previously attended child care.

Plan of Correction: The facility responded: The parents of each child will be asked to provide the missing information from their child's records.

Standard #: 22VAC40-185-440-A
Description: Based on observation it was determined that the licensee did not ensure that cots or rest mats shall be provided for children during the designated rest period. Evidence: 1. There were 7 children in the multi-purpose room during the designated rest period that did not have a cot or a rest mat. 2. Staff #3 confirmed that there were seven children that were not provided a cot or rest mat during the designated rest period.

Plan of Correction: The facility responded: The parents of each child will be asked to provide a map for the designated rest period.

Standard #: 22VAC40-185-550-C
Description: Based on a review of documentation and interview, it was determined that the facility did not ensure that the emergency evacuation and shelter-in-place procedures/maps shall be posted in a location conspicuous to staff and children on each floor of each building. Evidence: 1. The maps for emergency evacuation was posted in multiple locations within the facility. However the shelter-in-place location and the procedures for emergency evacuation and shelter-in-place were not posted anywhere in the facility. 2. Staff #2 confirmed that the post emergency map did not include the shelter-in-place location and the procedures for emergency evacuation and shelter-in-place were not posted anywhere in the facility.

Plan of Correction: The facility responded: The shelter-in-place location will be added to the posted emergency map. In addition, the procedures will be posted with the emergency map.

Standard #: 22VAC40-185-560-G
Description: Based on interviews, it was determined that the licensee did not ensure that when food is brought from home it is labeled with the child's name and date. Evidence: 1. The Licensing Inspector observed the lunch boxes for the 23 children in care and determined that none of the lunch boxes were labeled with the date. In addition, some of the lunch boxes did not have the child's name either. 2. Staff #3 confirmed that there were lunch boxes that were not labeled with their name or the date.

Plan of Correction: The facility responded: We will ensure the lunch boxes will be labeled with the child's name and the date buy using tape, a punch card or similar method.
