An unannounced monitoring was made at the center on 6/4/19 with local agency representatives in response to an incident reported to the inspector by the facility. Staff were interviewed on that date, records were reviewed, and the facility was observed. Additional staff interviews took place on 6/7/19 and 7/23/19. The inspection was completed on 7/26/19. The details of the investigation were reviewed in person on 7/31/19 with the owner of the corporation and the director of the facility.
