A complaint was received on 6/28/19 alleging a child was injured while in the care of staff at the center. The complaint investigation was started by interviewing staff on 7/3/19. The parent was interviewed on 7/9/19. Additional staff were interviewed on 7/25/19. Based on the interviews and documentation reviewed, there were no violations related to this complaint. This inspection was completed with the facility on 7/31/19.
