Comments:
An unannounced complaint inspection was conducted on 6/27/2019, in response to a complaint that was received by the licensing office on 06/20/2019. The inspector was on site from approximately 10:00 AM to 12:00 PM. Interviews were conducted with staff and members of the administrative team relating to allegations concerning staff qualifications and transportation services. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. In addition to the original complaint, there were also violations found during the inspection regarding special feeding needs and staff records. See the violation notice for more details. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Description: Based on a review of staff records, the center did not ensure that each staff record contains all required information. Evidence: The record for staff #2 (date of hire 4/15/16) did not contain name, address and telephone number of a person to be notified in an emergency.

Description: Based on a review of staff records, the center did not ensure that staff who drive a vehicle transporting children shall disclose any traffic violation that occurred five years prior to or during employment or assignment as a driver. Evidence: 1. Floor manager stated that staff #1 (date of hire 04/01/19) took children on field trip this week and several other times. 2. The record for staff #1 did not contain documentation that the staff disclosed whether or not she had any moving traffic violations during the fiver years prior to or during employment or assignment as a driver.

Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. Evidence: 1. In classroom #2 and classroom #3 chipped and peeling paint was observed. 2. There were two areas in classroom #1 with exposed crumbling drywall. 3. Classroom #1 had a broken outlet cover with exposed wires. 4. The hallway floor had a sticky purple substance at the entryway of classroom #3. 5. The refrigerator was leaking leaving a clear liquid puddled on the floor. The leak extended into the after school area approximately two feet. The wet floor posed a slip hazard. 6. In van #1 that transports children, three rows of seats had multiple holes exposing foam. 7. In van#1 that transports children, the rear side door was broken and unable to be opened. 8. In classroom#4, the oven used to cook meals for the entire center was on. Children were sent to the rest room next to oven without supervision.

Description: Based on observation and interview, the center did not ensure that when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely. Evidence: 1. Three highchairs were in use in classroom #1, the children in the highchairs did not have protective belts fastened. 2. The teacher stated that the seat belts were not in use at the time of inspection. 3. All children present were 12 months of age or younger.

Description: Based on a review of staff records, the center did not deny continued employment for one staff when a central registry finding was not obtained within 30 days of employment.
