Comments:
An unannounced, mandated renewal inspection was conducted today. The inspector arrived at 10:25 AM and departed at 11:45 AM. Eleven children were in care upstairs and fifteen children were in care on the first floor. Morning activities and snack service were observed. Meals and snacks are provided this summer by Thurgood Marshall elementary school. Nine staff records and a sample size of six children's records were reviewed. The program director reported that there were no children's injuries since the previous inspection. It was reported that no medication is administered to children in care. An exit meeting was conducted with the program director prior to closure of the inspection.

Standard #: 22VAC40-185-280-B
Description: Based upon observation and staff interview, the facility has not ensured that hazardous substances are kept in locked places using safe locking methods that prevent access by children. Evidence: 1. The bi-fold door to the stacking laundry machines does not lock. There was a gallon container of bleach and two containers of laundry detergent on top of the machines. All containers had warning labels and indicated to keep out of reach of children. 2. Staff 1 acknowledged the containers in the laundry closet and moved them to a locked location during the inspection. 3. The cabinet under the hand washing sink in the restroom used by the children in the downstairs classrooms was not locked. There was glass cleaner with a warning label and an unlabeled spray bottle of pink fluid in the cabinet. 4. The door to the upstairs break room was standing open. In the unlocked cabinet under the sink there was All Purpose Cleaner and Breath O' Pine, both labeled "warning and keep out of reach of children". Children were in care in two classrooms upstairs.

Plan of Correction: The facility responded with the following: Washing Liquid for clothes & bleach was located on top of the stackable unit. I had to get a step stool to reach it. It was removed during our inspection and placed in a locked cabinet behind a locked door. Never to be placed on top of the unit again. All containers were moved during the inspection All containers were moved from under the sink. The Breakroom door is to remain locked.

Description: Based upon observation, the facility has not ensured that when hazardous substances are not kept in original containers, the substitute containers clearly indicate their contents. Evidence: 1. Staff 5 showed the inspector the containers storing the chemicals used to clean and sanitize the changing table after diaper changes. There was a spray bottle labeled "bleach and water". Staff 5 stated that it was not bleach but another cleaning agent that she was unable to name. 2. There was a spray bottle of pink fluid under the sink in the bathroom used by the children. The bottle was not labeled as to the contents.

Plan of Correction: The facility responded with the following: All spray bottles will be label. The spray bottle with pink soap & water solution along with the bleach water solution will be lable.

Standard #: 22VAC40-185-340-A
Description: Based upon observation and staff interview, the facility has not ensured the care and protection of the children. Evidence: 1. At 10:35 AM, there were eleven children in care in a mixed age classroom of children aged sixteen months through three years. The mixed age group was in the classroom on the first floor far right. Staff 4 was the only staff person supervising these eleven children. Staff 4 was unable to identify the children in her care by name. Staff 4 also stated that the children in her care were aged twelve months through three years. The program director clarified that all the children in her care were at least sixteen months old. 2. Staff 3 was in an upstairs classroom with school aged children and a four year old child. When asked how many children she had in her care she responded that she had nine children. The inspector counted the children with staff 3 and determined that there was a total of only seven children in her care.

Plan of Correction: The facility responded with the following: We ensure the children reieve good quality care 1. The children in care range from sixteen months through 2 1/2 years old. The children were finishing up snack time. 2. Staff 3 was looking at the snack tray she had 2 extras. Encouraged to count often.

Standard #: 22VAC40-185-350-C
Description: Based upon observation and staff interview, the facility has not ensured that when children are in mixed age groups, the staff-to- children ratio applicable to the youngest child in the group applies. Evidence: 1. At 10:35 AM, there were eleven children in care in a mixed age classroom of children aged sixteen months through three years. The mixed age group was in the classroom on the first floor far right. Staff 4 was the only staff person supervising these eleven children. After ten minutes, staff 5, came into the classroom. Staff 5 had previously been on duty in the infant classroom until staff 6 relieved her so she could go into the mixed age classroom on the right. After fifteen minutes, the program director began coming in and out of this classroom, during which time the room came in and out of ratio. 2. The program director acknowledged the ages of the eleven children in the mixed age group and that three staff were needed to meet the five children to one staff ratio required.

Plan of Correction: The facility responded with the following: Director has held in-service training and also updated info to ensure staff learns the children names/birth days also, staff is encouraged to count often. ages. Staff revisited the child to staff ratios as per Standards.

Standard #: 22VAC40-185-560-H
Description: Based upon staff interview, the facility has not ensured that the catering service being used has current approval of the local health department. Evidence: According to staff 1, the food being served to the children is prepared and transported by Thurgood Marshall Elementary School. Staff 1 stated that she did not have documentation of health department approval or a health department inspection form for Thurgood Marshall school.

Plan of Correction: The facility responded with the following: We have obtained a copy of Thourgood Marshall health insp.

Standard #: 22VAC40-185-570-C
Description: Based upon staff interview, the facility has not ensured that the record of each child on formula contains the child's feeding schedule. Evidence: Staff 1 provided documentation in the form of a list indicating the formula that each child is to be served, however staff 1 was unable to provide documentation of each infant's feeding schedule.
