Technical Assistance:
New subsidy regulations, to include TB screenings and the Annual Health and Safety Update Training, were discussed with the owner during the inspection.

Comments:
A Subsidy Health and Safety inspection was conducted on 8/7/19 from 10:40am until 12:40pm to review supplemental health and safety requirements. At the time of the tour, there were 37 children in care with 9 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed eating lunch, resting quietly during nap time and participating in afternoon activities. Hand washing, restroom and lunch service procedures were also observed. Medication, transportation, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, children's injury reports and center postings were reviewed. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. One violation was found during the supplemental subsidy health and safety inspection and is identified on the violation notice.

Violations:
Standard #: 22VAC40-665-540-A
Description: Based on record review and interview, the vendor did not ensure that staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB) within the last 30 calendar days of the date of employment. Evidence: 1. The record for staff 1 (hire date: 7/26/19) contained documentation of a TB test completed on 12/3/18, which is more than 30 days from the date of employment. 2. The center owner confirmed that the TB test for staff 1 was not completed within the last 30 days of the date of employment.

Plan of Correction: Staff 1 will obtain a current TB screening. Going forwards, administrative staff will ensure all new staff have a TB screening completed within the required time frame.
